COVID-19 Event Guidelines
Questionnaire, Event Procedures & Release of Liability

The pandemic conditions are changing rapidly. Please be aware of the CDC guidelines as we will be following those closely.

Event Procedures

​

  • Please don't attend the event if you have a fever at or greater than 100 degrees Fahrenheit

  • We recommend sanitizing your hands (Hand sanitizer will be provided)

  • Mask may be worn in common areas at your discretion. (Masks will be available per request)

  • Please be mindful of social distancing guidelines and avoid over-crowding any areas

  • The CDC recommends that you get vaccinated

Release of Liability

Our organizations are following enhanced procedures to prevent the spread of COVID-19.

I understand that AIA Orlando cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.

By signing below, I agree to each statement above and release AIA Orlando from any and all liability for unintentional exposure or harm due to COVID-19.

Please fill out the Covid-19 Release Form no earlier than 2 weeks prior to the event(s) you'd like to attend.
COVID-19 Release Form
1. Have you or anyone in your household had any of the following symptoms in the last 21 days: Sore throat, cough, chills, body aches for unknown reasons, shortness of breath for unknown reasons, loss of smell, loss of taste, fever at or greater than 100 degrees Fahrenheit?
2. Have you had a positive-COVID test for active virus in the past 14 days?
3. Have you been in close contact with anyone who a has a confirmed COVID-19 diagnosis?
